This webinar, provided by the Institute for Healthcare Improvement, will offer an introduction that outlines what is an Age Friendly Health System and why is it important for delivering quality care for aging adults. This webinar will review the 4 M’s (what Matters most, mobility, mentation and medication) and offer suggestions for working with your health care organization to become more Age-Friendly.
